    Location, facility structure, & include all family involved in the operation. Be sure to list the size of herd, breed, coop membership, and/or where milk is sold. Include the history of the farm. Has it been handed down to you/nominee through many generations or recently moved/relocated to Illinois? Has your/nominee’s business structure or method of operation changed drastically over the years? Herd size changes, acreage, length of ownership, etc are pertinent pieces of information.
  • Part 2: Environmental Achievements

    Describe your/nominee’s efforts to be an environmentally friendly and a sustainable dairy farm. What programs have you/nominee participated in that help the public to see dairy farmers as good stewards of the environment? What are your/nominee’s goals for the farm in the area of environmental stewardship and sustainability? Be specific as to accomplishments and goals.
  • Part 3: Benefits to the Dairy Industry

    How have programs or improvements changed your/nominee’s farm, including environmental projects? What specific efforts can be shared that highlight these programs or improvements?
  • Part 4: Future of the Dairy

    What future plans or goals for your/nominee’s farm should be included in this application?

    Pictures showing the farm, the family, and/or conservation improvements or other environmental efforts to the dairy (before and after shots) are encouraged.
